Registration Forms

Things you should know...

• New families are encouraged to visit the preschool prior to enrolling their children. We want to meet you!

• Registration applications are accepted on  a first come, first served basis until enrollment capacity is reached.  

• Enrollment begins January 20 for current students who wish to attend the following school year.  Enrollment opens to the public February 3.

• All students are required to have the following two forms from the Ohio Department of Job and Family Services completed and submitted to the preschool prior to attendance.

  • "Child Enrollment & Health Information Form"

  • "Child Medical Statement"   

• Additional mandated forms may be required for students with special health concerns.

• There is an annual $35 registration fee for each family. (This is not based on each child enrolled, but on each family.)

• Tutition payments must be made online.  We will set them up for you. ACH payments do not incur additional fees. Credit card transactions incur an additional $5 fee per class to cover bank charges.

• All 4 forms (buttons on the right) must be completed and submitted to the preschool with the registration fee.
